Sounds like you have glazed either the pads or the disc or both.

Best way to tell is to feel them, if they feel smooth and shiney, then they have been glazed, it does happen over time.

You can either, try and use some emery paper and rub down both to get a rough surface, or buy a new set of pads and a disc.
