Hope this might help with your issue.

When I was competing in Prokarts, I made up special washers that went onto the crank before sliding on the clutch.

These were made with a "transition fit" on the crank diameter and with a radius machined on the inner face that matched the one that is ground into the crank. This meant that the clutch bolt could be done up tightly without any fear of the inner race of the clutch opening up.

I also made up special "thick" outer washers and was able to wire the cap head clutch securing bolt to the washer to prevent it falling out.
