over revving isnt a problem unless the piston to valves clearance is too small or the collet lets go.

If the valve was still attached to the collet then we can discount collet failure. That only leaves the piston to valve clearance...

# is the head gasket 1mm thick or more?
# has the cam timing been adjusted lately?
# was the normal torque used for the cylinder head joint?
# how bent was the valve stem (angle?)
To bend the valve would mark the piston, usually!
It is a puzzler...
