Interesting ???? Quote 'OTK axles are available without the 3rd bearing keyway holes' ???? The bearings DO NOT locate into any holes !! The holes to which I think you are referring are for fitment of an inboard sprocket carrier.
It does seem however, that these holes are where fractures tend to start from especially after use at PFI.
