It can be a number of things:-

1) Axle loose (as said by Zico and John) in it's bearings. Check the grub screws. If it is still m
oving after that, write again and I'll explain 'dimpling' the axle to prevent it slipping
2) the disk carrier may be moving along the axle. Make saure the mounting is grease/oil free and tighten it
3) The brake fluid could have absorbed water and be expanding when hot, thus jamming the brakes. Drain and replace all the fluid.
