TKM Clutch Washer Issue

I'm just in the process of switching from DD to clutched TKM (BT82). I assembled and fitted the clutch according to the excellent instructions on here (http://www.karting.co.uk/kandk/Tech/TKMClutch.html).

After starting the kart on the stand a few times the external thrust washer (that goes just behind the starter nut) had cracked. When I removed the nut the washer was in about 10 pieces.

I had tightened the nut to the correct torque (15 ft lbs). The washer was on the right way round (bevelled edge inwards). There was about 0.3mm float in the drum.

What could cause this? I guess it could just have been a faulty washer (I've now ordered a few spares). Could the starter have caused the nut to tighten up? Surely the engine doesn't have that much compression!
