* Insufficient (infrequent) or the wrong type of chain lubricant

* Particularly dusty/sandy or wet track. That can also occur through hitting too many kerbs with the chain, it throws concrete shards into the chain and thus files away the drive sprocket.

* poor chain alignment (left/right)

* poor chain adjustment (tight/loose)

* Old, nasty or poor quality chain

* Too few teeth in contact with chain: too small drive sprocket, too short chain, too large DRIVEN sprocket

* finally, it IS a 'consumable' like oil, fuel, brake pads, clutch shoes, etc.

* Finally.... finally..... a bad batch of drive sprockets........ that never got 'tempered' properly.
