ive had the teeth strip when i used a chain that was too short for the sprocket i was using,this meant that the angle the chain was at when in contact with the drive sprocket was too steep and it wasnt connecting to the teeth correctly.thats the way i saw it anyway and have never had the problem again

im sure there are other reasons people can mention but assuming you have the chain aligned and they arent buckled then its what comes to my mind.
