Sometimes the damage is as you describe and sometimes there is gouging.

Yes, the grubs do work loose, but on several occasions I have found the grubs tight but the axle damaged. Which either implies that the grubs come loose then tighten themselves up again, or the axle is 'not round' or that the axle is moving relative to the grubs.

I agree that the OP should replace the axle, my point was that if the axle has suffered damage, then it is worth checking the bearings too and consider replacing them, or cleaning and repacking them, if they contain metal fragments.
