If the key holes are worn,you may have a problem with the disc carrier type used, this may be allowing the key to move under loading, also is the carrier sitting central to key way holes, if predominatly one sided then this will encourage the key to wear the holes bigger on that side, more so on softer axles.Hope this helps.
