Might be worth running a new set of keys which might fit better to a new axle - less hammering affect??
Also not sure if you have problems with the redundant third bearing key holes propagating a crack, but axles are available without third bearing holes.
OTK axles with third bearing holes had issues at PFI, OTK axles are available without third bearing holes.
Apparently the exit kerbs which are to CIK standards can create a resonance in axles which can cause problems??
