Correct observations by Simon as usual.

The other thing I see is people popping the nut into the socket on the nut gun and then applying it to the stud.

This can cross thread the nut on the stud and it cuts a second thread into the stud, thus the gun ratchet operates and the 'mechanic' thinks the nut is tight.

Wind them on by hand first.

I can never understand why they keep operating the gun once the ratchet operates, as it will keep tightening the nut, which can result in the stud pulling out of the thread.

Also make sure the mating surfaces are clean.
