I worked in primary care (GP land) for a while and yes SimonS is quite right about the contract. What people fail to realise is that GP practices are not part of the NHS, They are private businesses who take a contract to provide services for the NHS (as a perk the GP gets an NHS pension though, but the other practice staff don't. Therefore practices can choose what they want and agree with neighbouring practices what a tariff should be.

As in all things, sometimes the paperwork and rules develop in to a new life form
