It's quite possible the led has gone wrong.

The component is only a minute bit of chip and it is possible it may have shorted inside.

Test that it is transmitting before you go chiseling the led out.

The battery voltage should be above 3.6 (3 x 1.2) but I wouldn't expect it to go much above 4.0 volts unless you overcharge it.

Why not let it discharge, then measure the voltage in its discharged state, then measure the voltage after 10 mins of chargeing.

It should recover to somewhere near 3 volts plus

If it is, then the battery is ok.
