Unfortunately, the answer boils down once again to 'politics'.

Organisers at every level are fiercely determined that their way is best and are often prepared to act against the interests of other people in order to get their way. Throw in the attractions of money and that determination to get their own way becomes sheer obstinacy.

They are helped by the apathy of the general karter, who expects that things are being done and is exceedingly willing to bitch about them not being right, but is remarkably absent in volunteering their services to get things done.

As a simple example, a frequent thread on here is "Reduce the number of classes" but the proponent of such an idea is rarely volunteering to see his chosen class shut down, or even willing to admit that any other class may have benefits that his selection doesn't.
