Problem with Slipping Rear Hubs and grub screws

hey guys.....got a slight problem, every time...or pretty much every time i come in after a session i find that my rear hubs have slipped along the axle by 10 or 20mm, ive tried to beast them up as tight as i can but they still seem to slip!...anyone got any suggestions on how to stop them from slipping? i also have a problem with grub screws coming loose, although i should get around that problem witha bit of loctite.
