That my friend is exactly how it's done.

Clarkson and co storm of with nothing but hand helds and in car mic & camera setups. The crew then take the car back filming all the external, beautiful landscapes and wot not before returning to carry out the months of editing.
The return journey is to setup all those fast panned shots and sequences where the car is accelerating away from the crew truck. It takes ages but it completes the show. This is quite often carried out by a team of drivers.

So in a way yes it's staged but the challenges are not, it's called entertainment.

That's also the reason many people believed Clarkson & co never drove the cars, but they very much do.
