It does happen if you don't use loctite on the threaded blade shaft at both ends - where it screws into the bellows housing (the black plastic bit) and the blade. Remove the PV from engine, Dismantle, clean, use new bellows and springs, don't forget the o ring and reassemble with a dab of loctite and set the length to be 75.0 (from top of black plastic top hat to inner most curve of the blade). Let the loctite set before bolting the PV back in place and DON'T get any Loctite in the housing where the shaft moves in and out.
