The karts are bunched up going very slow at the start. Lets say P19 starts pushing P17, so P17 pushes P15, P15 pushes P13 etc etc; depending on the nature of the circuit, sometimes this goes as far as the pole man. The P19 man just keeps pushing. To survive the P17 man has to do the same etc etc. A whole train of karts is now "loaded". Generally, the whole time you're wedged up against the rear bumper of the one in front you're ok. Of course, at some point you have to negotiate the corner. The guys at the front are going to start braking and the guys at the back are still pushing. The whole middle of the train of loaded karts gets pushed/spun/squeezed in all directions and the front of the train tends to go wide (into the outside pack). The guys further back will lift/back-off as the carnage starts and go straight up the inside. It's a risk for them, but with clutches they know that if they spin, they'll easily and quickly get going again and still be further up or no worse off than if they hadn't tried it.

When you take a circuit like PF, where you're still accelerating through the first corner, the issue is less problematic. The danger point being as you start turn-in. You're being pushed by a train of karts behind you, so your kart will tend to go sideways and wide, usually into the karts on the outside.

It doesn't just happen at the start, but that's when the right conditions are most prevalent (the back of the grid being able to accelerate before the poleman). Standing starts solve the issue because you never get that train of karts. Faster rolling starts solve the issue because everyone accelerates off the last corner and the field naturally spreads out.

If you watch at a circuit where the false start flag is after the first corner, you will see many fast clean starts get the false start flag. The field comes back round all slow and bunched up and the conditions for loading are perfectly setup.
