The issue with medicals is self certification works perfectly well in plenty of far more dangerous sports. So why do we put this barrier on karting? We are competing with other sports and losing.

SimonS, you don't need a license in IKR and I don't see the point in attempting to confuse matters. Yes you will be prevented from racing if you are incompetent but in reality such occurences are unheard of. Just like no one ever failed an arks test. I actually support licenses in principle but the MSA use them as a revenue stream, again creating an expensive barrier that other sports don't have.
