On the setup that I use on a Zip Storm / Lightning...
There are two aluminium 'grommets' that sit either side of the short open ended tubing on the bumper itself where it attaches to the frame. They should only fit one way, with the frame side grommet having a lip that will fit inside the chassis tubing. The bolt goes through: grommet-bumper-grommet with the cap head / flat hex head of the bolt on the outside of the rear of the bumper.
Then slip the rubber bung over the bolt - there should be a cone shaped recess at one end, which goes furthest away from the bumper.
Screw the cone shaped nut onto the end of the bolt, making sure that it fits into the cone-shaped recess in the bung. Lightly hand tighten at this point.
Repeat with the other mounting before pushing the whole bumper assembly into place by sliding the bungs into the chassis rails far enough to ensure that the lip of the metal grommet nearest the chassis engages properly with the inside of the chassis tube. It helps to spray some WD40 onto the bungs and inside the chassis rail before you do this.
Then tighten up the bolts from the outside - this should cause the inside 'cone' nut to be pulled towards the bumper, compressing the rubber and making it shorter and fatter, resulting in the bumper being held in place. (sometimes the cone nut can slip round, so make sure the bolt is free from dirt and has a dab of grease on the threads before assembling).
I use some cut down old brake cable, with the ends soldered, threaded through the bearing hangers and secured with cable clamps rather than plastic cable ties to secure the bumper setup. This allows you to leave the bumper 'loose', without fear of it falling off.
