That's a fair bit more serious than weld quality or tube gauge from what I can see. There have been problems with hydrogen embrittlement in steel of late, which are made a bit worse with welding.

Not going to pre-judge it, but I'd hope Intrepid are making a thorough investigation. If someone gets hurt, that's a lawsuit...does no-one any favours.
