Crankshaft alignment.

Checked one of our practice engines to find crankshaft end-float was hard to determine. Strip-down revealed a badly "out of true crank": no need for measurements, it could be seen by eye!
Given that the crank was "clocked and true" when assembled by myself, and we use appropriate locking tools to not "twist the crank" what could have caused this?
