i had same issue when returned afdter 13 yrs away.......i believe the arks counts as one signature, leaving another 5, however years ago it was signatures from 2 different tracks, now its just one track...as for the licence form itself....i cant say if they allow photocopied forms or not.....i'd have thought the money grabbing execs at msa would have thought of that and stopped it happening at all...maybe someone can answer that one better
