Inter61's idea is a new take on it and not a bad one either.
I'm with (seemingly) the majority here with added part. Its usually not the slow drivers who cause the problems, its the guys who's ambition overtakes their ability who cause massive accidents and the really fast guys sometimes push but usually without causing an accident.
Having watched a lot of european karting theres a reason the UK is generally good out there and it is because we start racing in heats and learn how to overtake. I've seen many a world champion unable to overtake usually only able to qualify on pole and sit there.
Anyway the reason for crashes on the first corner is CoCing. I know its a difficult job and spotting perps at the first corner is very difficult but if you've got almost no chance of being caught whats going to stop you doing it other then your own conscience. Drivers will start being very careful after being caught once or twice and the others will start be wary once they know punishments are forthcoming.
